Disability Support Worker

Position: Disability Support Worker

Summary of Role

Using a person centred and active support approach, the Support Worker (SW) will work as part of a team (which may include families) to provide support services to people with disabilities (Participants). The Support Worker will work with a range of plans (as appropriate) for each participant such as an individual support plan, health care plan, medication plan, meal plan, manual handling plan and will work within the rhythm of service for each house.

The Team Leader will demonstrate leadership and peer support to assist his/her teams to provide support to participants who are supported by BET to achieve positive outcomes. The Team Leader will actively support participant’s individual needs, support and promote their skills and abilities, provide support with activities of daily living, and develop, implement and monitor their Individual Plans.  The Team Leader will work with a range of plans (as appropriate) for each participant such as an individual support plan, health care plan, medication plan, meal plan, manual handling plan etc. to achieve and support optimum care outcomes for each participant.

Registered Nurse

Position: Registered Nurse

Reports to: Clinical Coordinator

Summary of Role

Using a person centred and active support approach, the Registered Nurse (RN) will provide basic clinical support to the Clinical Coordinator to optimise the health outcomes of participants. Through the provision of professional clinical health consultation, mentoring, training, and advice the Registered Nurse will work with SSWs and SWs to support a range of plans including individual support plan, health care plan, medication plan, meal plan, manual handling plan etc. to achieve the optimum care outcomes for each participant.
